We carried out an unannounced inspection of Outpatient services at St Luke’s hospital on 23 and 24 September 2025. St Luke’s Hospital is part of Bradford Teaching Hospitals NHS Foundation Trust. St Luke’s Hospital provides a range of services including outpatient’s and medical care. This assessment looked at outpatient services as part of routine inspection work in preparation for a trust level well led inspection. We rated outpatient services as good. The rating of outpatients has been combined with the ratings of other services from the last inspections. See our previous reports to get a full picture of all the other services at St Luke’s Hospital. The rating of St Luke’s Hospital remains good.
At this inspection we identified two breaches of regulations of the Health and Social Care Act 2008 (regulated Activities) Regulations 2014 in relation to Premises and Equipment and Good Governance.

We inspected outpatient services at St Luke’s Hospital on 23 and 24 September 2025. The assessment was unannounced and carried out as routine inspection work in preparation for a trust level well led inspection.
As part of our inspection we reviewed adult and paediatric outpatient services along with other outpatient activity within the hospital including allied health services, maxillofacial, dermatology, and cardiology services.
The service was rated as good because:
- Staff managed risk and involved people in their care. Staff followed effective systems and pathways and were trained in key skills.
- Staff followed evidence based practice and national guidance. Multi-disciplinary working and processes supported work to improve outcomes.
- Staff treated patients with compassion and kindness and responded to individual needs.
- Staff provided care in a way that was person centred and tailored to individual needs.
- Leaders understood the service and the forward vision. Leaders supported staff well.
However, we also found:
- The premises, environment and security were not always fit for purpose.
- Risk assessments and mitigations were not always clearly recorded.
The service was in breach of two regulations. One related to premises, environment and equipment and one for good governance.
An action plan will be requested upon publication of the final report.

St Luke’s Hospital provides general medicine wards for adults as well as rehabilitation and therapy services. The hospital also provides outpatient services for adults and children.
The Medicine (including older peoples care) Assessment Service Group (ASG); Outpatients and diagnostic imaging ASG at St Luke's was last inspected as part of a follow-up inspection of the trust on 11-13 January 2016 in response to a previous inspection conducted as part of our comprehensive inspection programme of the Bradford Teaching Hospitals NHS Foundation Trust in October 2014. Medical care (including older people’s care) was rated as good.
We received a number of anonymous whistle blowing concerns regarding safe care and treatment within the assessment service group for Medicine. We undertook an unannounced, responsive, focused inspection. We assessed key questions in safe, effective and well led.
